C/C/12. The Holloway Health Centre Microsoft Access Database

The Holloway Health Centre Database is created for the NHS group GP practice. This Database was created to help keep details of patients, doctors, hospitals, appointments, treatments, prescriptions and payments. There are six entities and six tables. Further it was produced six forms, which created to input details of patients, doctors, appointments, treatments, payments and hospitals. Furthermore, database is loaded. It was created one that minimises the database window and opens the switchboard form. The Main Menu was created using Accesss Switchboard add-in, and has the following menus: Patients Menu, Doctors Menu, Payments Menu, and Treatments Menu. The main menu will appear automatically, and the main window will be hidden, when the application is loaded. Also the project includes Data Flow Diagrams at high level, level 1, level; design an appointment card, prescription, and an invoice as three reports of the system and the menu structure that will help the Clinics staff use this system.
